I had a small chin implant 8 years ago. I am happy with the projection, but am now unhappy with the length. Would also like to get a more square implant. Is there a way to add length? Such as placing the implant slight lower? Also, is it much more complicated due to scar tissue from my current implant? Thanks!

From the pictures you have submitted, your chin looks very good. I would be careful about trying to get too perfect a result. That being said, the changes you are looking for are subtle and therefore may not be worth a second surgery. To answer your question: yes, there is always scar tissue formation after a surgical procedure. This scar tissue makes surgery in the same area more complicated and more prone to adverse outcomes. I frequently use filler injection for these kinds of subtle modifications where the results of surgical procedures have to be tweaked to get the perfect outcome. It is safer, easier, and cheaper. Filler injection is usually as effective and sometimes even more effective than replacing the implant.

The only problem I have with my nose is that the tip (Which has no bones..only cartilage) is pointing downwards. What I want to do is just make it more upturned. I’m not talking EXTREMELY upturned..just a bit more so it looks better and not as harsh.
I know non-surgical rhinoplasty is used to shape and contour the nose, but from what I’ve seen, it’s only used to fill in bumps.
Also, the fact that it’s not permanent is really annoying. If it is possible to make my nose more upturned, then I’ll ask for a permanent filler or something. Also, how much does this cost? Does the cost differ if a permanent filler is used?

The most dramatic results from non surgical rhinoplasty are with people who have exactly your problem. If you take a look at the before and after gallery in the reference, you will see what I mean. Various fillers are used for the procedure – I like to start with a temporary one like Radiesse and then proceed to a permanent one like Artefill once the person has experienced the effect and knows for sure that this is what they want. The issue with permanent fillers is that once they are in the skin, it makes surgery difficult. So, if you opt for Artefill that means that you are saying you are never going to have surgery. The cost does differ when a permanent filler is used. Artefill takes more sessions (3 to 6) to achieve the desired result and it is more expensive. I charge $2000 for the procedure, including the follow up sessions.

Hi Dr. Rivkin.
I too read some of the risks of using artefill. I have heard that since aretefill is fairly new, some doctors will not use it until they can see the long term effects (5-10 years down the line).
I have heard some people having great results with you with the semi-permanent fillers for their non surgical nose job. I had a rhinoplasty done, and although I like my results, there are a few minor changes I would like to make in the areas close to the tip. I as considering artefill to achieve permanent results.

Just a few questions:
Do you still use artefill for the non surgical nose job?
Over how many patients have you used aretefill on for the non surgical nose job?
Are they really permanent, or only permanent for up to 5-10 years. (If they do last only 5-10 years.. that means they do eventually dissolve correct?)
Will aretefill migrate?
If I don’t like the result, what can be done to remove/correct this?
What is the price range for this procedure?

Thank you in advance for your help!

hi,
yes, i use artefill with a great success rate for the non surgical nose job procedure. the only trouble is that people have to come in for multiple touch up procedure, as the artefill does tend to settle down after the injection.
i have done about 100 artefill nose procedures and everyhting has gone really well – the only frustration has been the multiple (3 to 5) touch ups needed before permanent results are achieved.
the material does not dissolve, so the results should really be permanent. i do not think that it is a 5 year procedure, but i can’t tell you for sure that you would not need a small touch up after a few years. however, i have not had to do these long term touch ups and i’ve been using artefill for 3 years now.
artefill will not migrate.
$2000 – no charge for touch ups.
hope that helps,

Hi,
Nope, you can do anything you want!
Afterwards, I wouldn’t take Aspirin or Ibuprofen for a couple of days. Also, I wouldn’t manipulate the area very much for a few days, but you can touch it and wash your face – that’s all okay. Keeping the area clean is important for the first day or two especially.
Hope that helps
