The midface is probably the most important area of the face that “gives away” aging. The loss and descent of facial fat volume in this area is one of the first signs of facial aging. You can’t really lift the area non surgically, but there are significant things that can be done to improve the appearance. First, the tear trough and under eye area can be re-filled with Restylane and Radiesse. This is usually the first sign of aging and filling this area usually gives a dramatic improvement. 10 years in 10 minutes is not an exaggeration. Replacing the definition and prominenece of the cheekbones is also important and can be done with Radiesse or Sculptra. Volumizing the cheekbone area changes the shape of the face back to the heart shape of youth – wider at the cheekbones and narrowing at the jawline. Refilling sunken cheeks and temples is the next important step and Sculptra is a great injectable for that. The side benefit of Sculptra is that it improves the quality of the skin where it is injected. The pores are smaller and the skin becomes thicker and firmer. Filling deepened nasolabial and marionette lines with Juvederm or Radiesse is, of course, part of the equation. Hope that helps.

Notice the amazing before and after image, but don’t overlook (its easy to do so) the tiny print, partially obscured disclaimer that reads “patient benefited from the face, neck and skin firming procedures and laser skin treatment”

In other words, this patient had a whole bunch of procedures done (and her after pictures were probably photoshopped, but that’s another story) and her result was good. We (LifestyleLift) know that our procedure was only part of the reason she looks better, but we will take all of the credit and are betting that you, consumer will not read the fine print. You, consumer, will then go get a lifestyle lift and be disappointed by mediocre, short lived results. But we, LifestyleLift, don’t really care because we are fooling so many people that the next sucker is just around the corner. So you went in for a cheap, mini facelift, but then had to go to an experienced plastic surgeon for a full on revision facelift to correct the bad results that costs five times what you wanted to spend and will probably still have suboptimal results because revisions are always very tricky.

By the way, I don’t do facelifts and my practice is devoted to keeping people away from the operating room for as long as possible. I have no economic axe to grind – I’ve just seen too many disasters from this procedure.

If a knife is going to meet your skin, make sure that the hand behind the knife is as experienced and legitimate as possible!

Try non surgical options first

You have mild laxity in this area, so I think you would get good results from one of the radiofrequency skin tightening devices on the market. I have been working with Thermage for many years. Results vary between individuals, but should quite good for someone with mild laxity like yourself. The other benefit to Thermage in that area is that you can treat the jaw line at the same time. Skin tightening around the jawline will give it more definition which makes one’s face look younger. Ultrasound based devices are also on the market, but they are new and I’m not sure of their efficacy.

Dear Dr. Rivkin,

I got Radiesse in my mid face four months ago to relief the nasabial folds.
The effect has not yet worn off. I heard it last 1-2 years.
But now I inherited money and want a midface lift and permanent fat grafting to my mid face.
Can a plastic surgeon remove the Radiesse from my face or do I have to wait till it has worn off?
In other words: how long do I have to wait till I can get a real mid face lift or cheek lift surgery?
I am afraid that if the Radiesse is still in my face when the PS does the surgery that he might not lift the area where the Radiesse is in and then the face will suck when the Raddiesse wears off 6 months later…

Thanks you for answering.

hi
i agree with you – i would wait until the radiesse wears off before going for a surgical lift. unfortunately, there is no way to take it out. your best bet is to wait about a year after the procedure was done.
hope that helps

I am much too young for a facelift of any kind. But in March I had a chemical peel done to my face that left my skin with a rough texture/orange peel look. Do you know about this side effect? I have tried many other procedures such as micro-dermabrasion, smoothbeam and creams to fix or help my skin but nothing is working.

When I take my skin and stretch it back a little, my skin looks smooth again though. So this is my I am considering a facelift. I am most interested in focusing on the apples of my cheeks and a bit below that, towards my mouth. What are your thoughts on this?

I was also considering a rhinoplasty as a have a bulbous nose and would possibly like to thin it out very, very slightly. My main concern is the facelift, but the reason I mention this is because I was curious whether having a rhinoplasty along with the face lift, would allow you to tighten the skin better in the areas I desire.

Thank you for your time.

I do not think that a facelift will help you.

The problem that you are describing has more to do with the texture of your skin than with overall skin laxity. I have heard of this kind of thing happening, but I do not really use chemical peels, so I have not experienced it among my patients.
It takes a lot of energy to change the texture of your skin once this kind of thing has happened. Microdermabrasion and creams will not help. Smoothbeam is an okay choice, but even that is not potent enough, in my opinion, to change the topography of your facial skin. The only laser that I have seen to really have an effect is the active fx. I would suggest that you do a bit of research on that laser – it should help tighten your skin a bit and smooth out the orange peel effect.

Regarding your nose, it sounds like you would be a candidate for non surgical rhinoplasty. It would be a shame to undergo surgery for a subtle effect such as the one you’re talking about. It can be achieved with a few steroid injections.
